No titer test is required for departure from Armenia; entering the EU/Schengen area follows the EU's general rule for "unlisted" countries.

Rabies vaccination annually, no less than 30 days before departure.

Not required for departure from Armenia itself.
Documents
A veterinary certificate is issued by the state vet service at your place of residence and, at the departure airport (Zvartnots), by border veterinary control.
Entry notes
Check with the airline — some carriers from Yerevan connect through transit hubs with their own animal-transit rules.
